Acknowledgements

The AIHW thanks and acknowledges the large contribution by staff from a range of organisations in providing datasets and advice in the production of this report. These organisations are:

Ethical approval for the study was provided by the AIHW Ethics Committee and the Departments of Defence and Veterans’ Affairs Human Research Ethics Committee. The NCIS analysis approval was provided by Justice Human Research Ethics Committee, Coroners Court of Victoria, Tasmanian Coroners Court, and Western Australia Coronial Ethics Committee.

Amendment

10 March 2023 – The downloadable excel ‘Data tables’ has been updated for a minor correction. In addition, a minor typographical update was made to the report text.
